Petition to Graduate
Instructions
Please download and complete the petition to graduate form which matches your program area. You must place check boxes in the grid indicating in which quarter you completed each required class. Also please indicate which classes were taken to fulfill the elective requirements. For those classes which you have not yet taken, please check the box indicating the quarter in which you intend to take the class. Every course listed on the petition must have a check mark indicating when it was taken or when it will be taken.
The completed petition may be faxed in to the MSEd office at 847-467-2495. Your advisor will review your completed form and compare it to your transcript in order to verify that the information is correct, and will then sign it.
If this form is not submitted by the deadline (notifications are sent to the MSEd Listserv in early January), you will not be able to graduate at the time you had originally planned, and will need to wait until the next available graduation ceremony. There are only two graduation ceremonies each year: one in June, and one in August.

Elementary and Secondary Teaching students should keep in mind that these petition forms are only intended to verify your graduation requirements. Your certification and endorsement requirements are handled separately, and are reviewed by the certification manager.
